First you must register with DWI Services. Afer your registration is received and approved, you will be sent an email with a password you will use to continue the application process. (Please make sure the email address you include on your registration form is a working email address.)

1. Download Registration Form (opens as a new tab).
   
2. Print out the registration form.
   
3. Fill out your registration form.
   
4. Write a check for $250, made payable to: DMH/DD/SAS.
   
5. Stamp and mail your registration and check to:
