This system allows for automated processing of 508 forms required for all convicted offenders to ensure completion of DWI services before they may have their drivers’ license reinstated.

The forms will be sent directly to the state office, electronically, for review and approval. There will be a record of each transaction for future reporting, monitoring and evaluation.

Each agency must have a facility representative, who is given authority to access the system and maintain responsibility for agency contact information and user access. The rep can set up additional agency users, if needed, to complete 508 forms online.
