e508 Rejections by DMV
"Treatment Completions" that have been rejected by DMV can be corrected.

1. Customer Number: If the Customer Number is incorrect the "Assessment" section of the E508 needs to be completely re-entered using the correct Customer Number then resubmitted to the state. 2. Arrest Date: If the customer’s Arrest Date is incorrect the “Assessment” section of the E508 needs to be completely re-entered using the correct Arrest Date then resubmitted to the state.

3. County of Arrest: If the county of arrest is incorrect the “Assessment” section of the E508 needs to be “Updated” then resubmitted to the state.

4. Conviction Date: If the conviction date is incorrect the “SA Treatment” section of the E508 needs to be “Updated” then resubmitted to the state

5. Conviction Date: If there is NO conviction date indicated on the MVR there is a good chance that the court has not sent the record of conviction to DMV. Providers should contact the Clerk of Court to get the record of conviction forwarded to DMV. Once the record of conviction has been sent to DMV the Provider should then “Update” the conviction date in the “SA Treatment” section of the E508 and re-send the “SA Treatment Completion” to the state.
